SEXUAL DYSFUNCTION
As a result of Pelvic Floor Dysfunction, Prostatitis, and/or medication, men may experience the following sexual dysfunctions:
HARD FLACCID:
- Pain at pubic bone
- Pain or tightness between scrotum and rectum
- Pain or numbness in penis
- Penis in semi-rigid state, not erect, not flaccid
- Scrotal constriction/tightness to body (under wear are uncomfortable)

ERECTILE DYSFUNCTION (ED) AND PREMATURE EJACULATION:
Erecticle Dysfunction and Premature Ejaculation may be a sign of pelvic floor weakness, however, ED can also be as serious medical condition. One or more of the following comorbidities may contribute or cause erectile dysfunction:
- Cardiovascular disease
- Coronary Artery disease
- Sickle Cell Anemia
- Diabetes
- Chronic Periodontitis
- Peyronie’s Disease
- Epilepsy
- Surgical events
OTHER REASONS FOR ERECTILE DYSFUNCTION:
- Premature ejaculation
- poor nutrition, low BMI
- Obesity, high BMI
- Low testosterone
- Illicit Drug use
- Smoking Depression
- Spinal Lumbar Pain
If the man is experiencing erectile dysfunction yet can wake with a morning erection there is usually a mental component to their ED. Physiologically everything is working if an erection can occur when the man is not under stress (first thing in the morning). Therefore, the ED may be due to performance anxiety or lack of sexual desire. The first step should be to get testosterone levels checked.
There has been a new onset of ED with men under the age of 40. This is thought to be a precursor or predictor of cardiovascular disease later in life especially when the following are present: premature ejaculation, poor nutrition, low BMI, low testosterone, smoking, and illicit drug use. If you have erectile dysfunction you should make an appointment with your physician to ensure it is not an underlying medical condition.
PREMATURE EJACULATION:
If this is due to pelvic floor muscle weakness, the following techniques can be utilized to improve performance:
- The use of a numbing topical agent or condom to decrease sensation and allow for preemptive control
- Kegel exercises (See Kegels)
- Stop/start maneuver. At mid-level excitement, withdraw, start again, repeat. You can also apply pressure to perineal body
- Circular thrusting–try changing stimulation on penile shaft and glans
Masterbating often while practicing these techniques will improve control.
